What to bring: A copy of your medications, insurance cards, completed questionnaire that was either mailed or you downloaded from the website, contact lens Rx and glasses you are currently wearing.

New Patient Referrals

If you have been referred to our office by another professional, please please let us know this when you call to schedule an appointment. Having referral information sent to us ahead of time helps us to prepare for your visit. After you or your child have been examined and we have discussed our findings and recommendations, a written communication will be sent back to the individual who referred you. If the referral source is another optometrist, we will see you for specialty services and you will return to your doctor for your primary eye and vision care and glasses. We look forward to seeing you.
